Hazel Jakubczak, (nee Howes), age 83, late of Hammond, IN and formerly of the East Side, passed away September 28, 2020.  Beloved wife of Ronald for 64 years.  Loving mother of Stanley (Karen), Terry (Deirdre), Thomas (Laura), Kenneth, Kathryn (John) Surwillo and Ronald (Raquel) Jakubczak.  Devoted grandmother of 14 and adored great grandmother of 5.  Dearest sister of Heather (Peter) Box of England.  Fond aunt of many nieces and nephews.  Visitation Friday 3-8pm.  Funeral services Saturday, October 3, 2020 at 10:00am at Elmwood Chapel, 11200 S.
